Most UHC employer PPO plans cover 20 chiropractic visits per year, though some plans allow 26 or 30. UHC Navigate plans typically provide 60 combined visits for physical therapy, occupational therapy, and chiropractic. CalPERS UHC HMO covers 20 visits per year combined with acupuncture. All visits are subject to medical necessity. We track your remaining covered visits and notify you as you approach your limit.
For standard employer PPO plans (Choice, Choice Plus), no prior authorization is required for chiropractic care. UHC Medicare Advantage plans in California began requiring prior authorization in early 2025, but after industry pushback UHC modified the policy: the first 6 visits within 8 weeks are covered without clinical review, and care beyond 6 visits is assessed for medical necessity. This applies to Medicare Advantage only — not your employer PPO plan. We handle any authorization requirements on your behalf.
Yes. Chiropractic care is a qualified medical expense under IRS guidelines — you can use your HSA or FSA to pay for copays, deductibles, and out-of-pocket costs regardless of your UHC plan type. We accept HSA and FSA cards directly at our office.
UHC Choice plans have severely limited or no out-of-network chiropractic benefits — meaning you'd pay the full cost out of pocket. UHC Choice Plus plans do include out-of-network coverage, typically at 40–coinsurance that varies by plan after your deductible.
